Serum Vitamin D and Parathyroid Hormone Levels in Patients with Osteoporotic Hip Fracture

Abstract
Objective: The purpose of this study was to examine relationship between serum vitamin D and parathyroid hormone (PTH) levels in patients with osteoporotic hip fracture. In addition, bone mineral density (BMD), body mass index (BMI), albumin, osteocalcin, and calcium intake were measured and analysis was performed.

Methods: There were 193 patients who had hip fracture by mild trauma from 2005 to 2009. Some patients who had preexisting conditions affecting vitamin D synthesis and bone metabolism were excluded. Finally, 119 patients were chosen as study subjects. Patients¡¯ serum vitamin D, PTH, albumin, osteocalcin, calcium intake, BMD, BMI were examined and compared with another. Also compared with control group.

Results: The mean levels of serum vitamin D, PTH, and osteocalcin were 21.6 ng/mL, 33.6 pg/mL, 19.0 ng/mL in patients and were 33.9 ng/mL, 31.6 pg/mL, 25.4 ng/mL in control. Also, average of BMI, albumin, calcium intake were 22.6, 3.3 g/L, 484.4 mg/day in patients. There was 82.4% of patients decreased vitamin D below 30 ng/mL and 21.0% of patients decreased below 10 ng/mL. Only 9.8% of patients with vitamin D below 30 ng/mL were above 65 pg/mL in PTH.

Conclusion: There was 82.4% of hypovitaminosis D in patients with osteoporotic hip fracture. However, parathyroid hormone did not increase significantly. We conclude that serum vitamin D reduction may be one of risk factors in osteoporotic hip fracture.
